HAVE YOU BEEN BOOKED FOR SURGERY AT STIRLING HOSPITAL?
Your admission paperwork has never been easier – activate your account in a few easy steps with our online patient registration system, Personify Care.
Enter your mobile number to get a link sent to your phone to register with Personify Care. You can also select to register via your email address.
You can select to register as the patient, or if you need some help you can elect for a proxy to register on behalf of the patient. Next create a password and select Register.
Fill in your details (or those of the patient if you are registering on their behalf) and select ‘I accept’ to allow Personify Care to share your health information with your healthcare provider.
You will then be taken directly to your account and will land on the ‘To Do’ list screen. You will receive reminder messages as you go along your admission journey. We will contact you if we need any more information.
Alternatively you may receive a message from Stirling Hospital inviting you to register via a link and this will follow the same process as above.
To minimise delay, please ensure that you have completed your online patient registration at least 10 days prior to your admission date.

Admission times are usually arranged by your doctor, however we may contact you to alter your admission time.
Pre-Admission Information
PRE-ADMISSION SERVICE
A pre-admission service is provided by the Hospital for patients undergoing surgery. Our pre-admission nurses will review the medical history information you provide prior to your admission. Depending on the type of surgery and anaesthetic you will be having, you may be contacted by one of our nurses to discuss further the information you have provided to us.
You will be contacted prior to your admission date if you need to attend our pre-admission service. Your doctor or pre-admission nurse may also require you to undergo blood and / or other tests before admission.

PAC bookings are available as a telephone consultation or as a face to face appointment.

For Department of Veterans’ Affairs (DVA) Patients
Department of Veterans’ Affairs (DVA) Veteran Liaison Officer (VLO)
At Stirling Hospital, we have a nominated staff member who is responsible for the duties of a VLO who will provide support to our DVA patients and their family / carer during their admission to hospital to enhance the treatment and service provided to veteran patients.
These duties include:
- acting as a single point of contact to resolve any concerns and problems
- providing pre-admission and discharge planning support
Stirling Hospital and DVA acknowledge that the VLO plays a vital role in ensuring that DVA patients are supported during their hospital stay. A VLO will assist DVA patients as they navigate hospital services on admission and communicate with them when needed to monitor their progress throughout their hospital stay.
